The Clinical Social Worker-LCSW job assesses and triages patient and family needs.
Processes referrals and schedules clinic appointments.
This role assists patients and families with feeling comfortable throughout clinic appointment.
This role assists with interviewing patient and family members.
This role ensures mental health resources are available to all patients and family members.
This role ensures families follow up with recommended services.
This role also assists patient and family with health insurance issues and difficulties obtaining prescribed medications.
The Clinical Social Worker-LCSW makes appropriate referrals.
Encourages use of trauma-informed practices.
This position ensues law enforcement and Child Protective Services (CPS) understand medical terminology and recommendations.
This position also acts as a liaison between CPT, law enforcement and CPS to ensure ongoing collaboration.
The Clinical Social Worker acts as a liaison between medical team members.
This job assists with developing best practices and protocols.
This job also maintains medical and legal documentation.
Licensure, Certification, or Registration Requirements for Hire:
Licensed Clinical Social Worker (LCSW) in the State of Virginia required.
Licensure, Certification, or Registration Requirements for continued employment:
Licensed Clinical Social Worker (LCSW) in the State of Virginia required.
Experience REQUIRED:
Minimum of five (5) years of social work experience.
Experience PREFERRED:
Previous work experience providing case management and discharge planning in an acute care setting or outpatient setting.
Previous work experience providing individual, group, and/or family therapy in an acute care setting or outpatient facility.
One (1) year in a lead role.
Education/training REQUIRED:
Masters of Social Work Degree from a Council on Social Work Education (CSWE) accredited program.
